The front office is still in need of student forms from many families. These were mailed in the summer packets. If you have questions or need new copies, please email Chris Harris. All forms are due by August 31st.

Carpool Permission Form – Only fill out if you have other family members or friends who have permission to pick up your child.

Photo Permission Form – Each student needs one on file.

Field Trip Permission Form – Each student needs one on file.

DSS Form – Only Preprimary and Primary students need this form in their file

Student Medical/Health Plan Form – Each student needs one on file.
